Eye Exam Instruction
Your eye exam will usually take 1 to 1 and a half hours. Your eyes will most likely be dilated. We strongly recommend that you bring a driver.
Please bring your glasses that you usually wear and any newer or back-up glasses with you for your exam, even if you do not usually wear them. If you wear contact lenses, please wear them in, and let us know what solutions you are currently using.
Bring all of your eye medications.
Please send or take a signed note requesting your old eye records to the office of your prior eye doctor. These should be sent to our office before your exam or brought with you to your appointment.
If you have a driver's license form, please bring it with you for your appointment.
If your insurance requires a referral, please contact your primary care provider.
Please look through and complete the enclosed information sheets and be sure to bring them with you to your appoinment.
Please bring your insurance cards and co-payment to your appointment.
